Tradition holds that on the night of October 22, 1598, a pilgrim left a bag in the castle courtyard atop the promontory containing a small statue of the crowned Madonna with the Child. The Raffa brothers, guardians of the castle, recognized the pilgrim as the venerated Saint Cono Navacita, who in his lifetime had pointed to that very peak as the place to build a sanctuary. After the first miracle attributed to the Virgin, the statue was welcomed into the church on October 22, 1600, and the solemn festival with a market and fair was established. The Sanctuary of Maria Santissima stands about 80 meters high, where a castle once stood guard against pirates, and is reached only on foot by climbing a long staircase of 230 steps.
The heart of the festival is the dawn pilgrimage: at the first light of day, an uninterrupted flow of faithful climbs to the Sanctuary, facing the staircase in silence and devotion. Holy Masses are held on the mountain throughout the morning, culminating in a solemn celebration; this is followed by the evocative Procession of the Virgin, which marks the descent of the statue from the mountain to the town and the Cristo Re Church. In the afternoon, the grand city procession accompanies the statue along the main streets of the center for the blessing of the city.
Alongside the religious dimension, the festival reveals its more popular side with a large street fair that winds through the town streets, featuring hundreds of stalls, local products, and handicrafts, as well as a funfair for families. The days are accompanied by the Capo d'Orlando City Band and evening concerts in Piazza Matteotti, while the festival traditionally closes with a fireworks display on the Andrea Doria seafront. Celebrations extend into the preceding days with the preparatory novena and continue until the octave on October 26th.

The Sanctuary can only be reached on foot via a staircase of 230 steps: wear comfortable shoes. Free admission to all celebrations and square events. During the festival days, the center is closed to traffic: it is advisable to park on the outskirts.
