The Saints Cosmas and Damian Patronal Festival is the most important religious celebration in Alberobello, located in the heart of the Itria Valley, in the province of Bari. For nearly four centuries, the city of the trulli has paused to honor the two healing saints, protectors of doctors and the sick: a centuries-old tradition that reached 389 years of history in 2025, deeply rooted in the community since 1637. For four days in late September, the historic center transforms into a grand stage of faith, light, and music.
The religious heart of the festival revolves around the Basilica of the Saints Cosmas and Damian, a destination for pilgrims arriving from neighboring towns and across Puglia. Devotion is expressed through Eucharistic celebrations, which begin at dawn, and above all in the two solemn processions of the saints' statues, carried on the shoulders of confraternity members and accompanied by civil and military authorities, local mayors, and a vast crowd of the faithful.
Alongside the spiritual dimension, the festival offers the spectacle typical of major patronal feasts in Southern Italy. Artistic illuminations light up Piazza del Popolo, Corso Vittorio Emanuele, Piazza Curri, and Largo Martellotta, creating light-filled scenography among the trulli. Brass band concerts enliven the streets of the center according to ancient tradition, while grand fireworks displays close the days with pyrotechnic competitions highly anticipated by visitors.
The Saints Cosmas and Damian Festival is not just a religious rite, but a moment of strong collective identity for Alberobello. The ancient fair that accompanies the festivities, live music, food stalls, and the community atmosphere make this festival one of the most authentic experiences for discovering the folk culture of Puglia. Organized by the Alberobello Patronal Festival Committee together with the Municipality, it attracts thousands of people every year to the evocative setting of the trulli, a UNESCO World Heritage site.

Alberobello can be reached by car via the SS172 dei Trulli road; by train via the Ferrovie del Sud Est (Bari-Taranto line), with the Alberobello station just minutes from the center. The nearest airports are Bari-Palese (approx. 60 km) and Brindisi-Casale (approx. 70 km).
The festivities are concentrated in the historic center, between the Basilica of the Saints Cosmas and Damian, Piazza del Popolo, Piazza Curri, Corso Vittorio Emanuele, and Largo Martellotta.
The festival takes place in late September and attracts many pilgrims and visitors: it is advisable to book accommodation in advance and arrive early to witness the processions. Participation in religious rites and brass band concerts is free.
