The solemn celebrations in honor of Saints Cosmas and Damian are the most important event of the year for Tocco Caudio, a municipality of a few hundred inhabitants in the Province of Benevento, in the inner Sannio area of Campania. Traditionally held in the final days of September, the patronal feast mobilizes the entire community and attracts thousands of pilgrims from neighboring towns: it is not just a devotional event, but—as the Municipality of Tocco Caudio itself emphasizes—an appointment of great historical, social, and cultural value, capable of bringing emigrants back home and filling the village streets with voices.
The heart of the festival is the Sanctuary of Saints Cosmas and Damian, on Via Carlo di Tocco, built around the first half of the 18th century around a pre-existing chapel and expanded over the centuries thanks to popular devotion. The building, with a rectangular plan featuring a central nave and two side aisles, houses a painting by an unknown artist depicting the Virgin with Saints Cosmas and Damian and the Child: a simple, luminous place that can become crowded with the faithful during the festival days. Cosmas and Damian, martyr physicians from the East, are invoked here as healers, and their cult spans the entire South of Italy.
The most intense tradition is that of the so-called "Scauzi a Santo Cosimo": a penitential walk that devotees—the so-called "battenti", once numbering in the hundreds—perform barefoot to reach the sanctuary, often starting before dawn. It is a silent, intimate rite passed down from father to son, representing the most visible sign of how deep the devotion to the Physician Saints is in this part of the Caudine Valley. Those arriving from outside observe it with respect: it is not folklore staged for visitors, but a promise kept.

The musical evening has made Tocco Caudio known well beyond the provincial borders: the artistic direction curated over the years by local artist Tonino Fusco, known as "Gitano", has brought protagonists of Italian music to the festival stage, such as Gigi D'Alessio (2017), Paola Turci (2018), I Nomadi (2022), and Andrea Sannino (2025). The concerts are traditionally free to enter, as are all the festival events.
A visit to Tocco Caudio during the festival is also an opportunity to discover one of the most evocative places in the Sannio. The ancient historic center, of Lombard origin, was completely abandoned after the 1980 earthquake and can now be reached on foot near the San Gaudenzio area: a true ghost village, with the remains of the church of San Vincenzo. Just above it, the Pietra di Tocco dominates, an imposing limestone boulder that served as an ancient lookout, with traces of Angevin-era walls and a small church dedicated to Saint Michael. The entire territory falls within the Taburno Camposauro Regional Park, on the eastern slope of Mount Taburno, among chestnut groves, olive groves, and vineyards.
The festival is traditionally held in late September, around the liturgical feast of the Physician Saints (September 26): in recent editions, the period has been September 25-28. The detailed program of religious and secular events is published every year by the Municipality of Tocco Caudio in the documents section of the official website and distributed via posters displayed in the village: it is therefore advisable to check the exact dates in September before setting off.
The 2026 edition of the patronal feast of Tocco Caudio, in the Province of Benevento, has not yet been officially scheduled. However, the recurrence is annual and deeply rooted: the sanctuary of the Physician Saints on Via Carlo di Tocco, the solemn masses, the processions, and the penitential walk of the "Scauzi a Santo Cosimo" return every year around September 26, the liturgical day of the two saints.
As per custom, the detailed program—religious services, procession times, performances, and the closing concert in the square—is published by the Municipality in the documents section of the official website and posted in the village, usually a few weeks before the event. This entry will be updated as soon as official information becomes available.
Tocco Caudio is located in the Caudine Valley, on the eastern slope of Mount Taburno, in the Province of Benevento. By car, you can arrive from the Appia state road (Benevento-Caserta) by exiting towards Vitulano, Foglianise, or Campoli del Monte Taburno; from Naples, take the Caudine Valley route via Arpaia-Montesarchio. The nearest train station is Benevento, about a half-hour drive away; the reference airport is Naples Capodichino.

During the festival days, the village gets crowded: it is best to arrive early and leave your car in the parking areas at the entrance to the town, then move on foot. The streets are steep, so comfortable shoes are recommended. For those who want to follow the "Scauzi" walk, you need to get up very early and maintain a discreet attitude: it is a devotional rite, not a show. For overnight stays, you can look at the agritourisms in the Taburno Camposauro Park or in Benevento and Sant'Agata de' Goti.
